
This form is required for the troop leader or co-leader to complete in any of the following circumstances:
This completed form should be submitted to Council Staff 72 Business Hours before the activity. This will inform the Council of your plans and will insure they have the necessary information in case of an emergency.

NOTE: If an activity is over 24 hours and/or if it involves cooking, Basic Outdoor Training is required and an Application for Troop/Group Overnight Stay or Horseback Riding form must be completed instead of the Special Activity Plan.
The Add-A-Girl Patch Program was created to encourage Troop Leaders to bring new girls into their troops. These new girls can be from either a Girl Scout’s referral of a friend or from the Service Area Waiting List!
All active Troop Leaders, Co-Leaders and Assistant Leaders are eligible to earn the patch.
Fill out the Add-A-Girl form and bring it to the Low Desert office with the Girl's registration. A patch or charm, depending on what you need, will be issued to you at that time.

The Add-A-Friend Patch Program was created to recognize girls who refer
their friends to Girl Scouting.
Every registered Girl Scout is eligible to earn this patch.
Fill out the Add-A-Friend form and bring it to the Low Desert office with the Girl's registration. A patch will be issued to you at that time.
